Care Corner Cluster Support provides social support and intervention for vulnerable seniors with no or weak family support. We help seniors to age in place and remain integrated within the community for as long as possible.
These may be seniors who are frail, have multiple or complex care needs, experiencing some form of isolation socially or emotionally, or have limited access to resources in areas of social, financial, medical and housing support.

Ways We Support
Key features of Cluster Support include home visits to provide case management, emotional support, counselling, coordination of social and health care services, monitoring and support for caregivers to meet seniors’ social and health care needs.
Basic Care Coordination
We ensure that the seniors’ care needs are adequately addressed e.g. meal delivery, medication/ medical appointment reminders, home maintenance, personal hygiene, social and recreational activities.
Collaborating with healthcare providers and community partners, we ensure that seniors and their caregivers receive sustainable help.
Monitoring and Support
During regular home visits, we monitor the overall health status and safety of seniors e.g. daily blood pressure monitoring.
Medical Escort
Cluster Support clients who require help to attend their medical appointments may be supported by our trained medical escort volunteers.

Counselling
Through our Gerontological Counselling programme, we help seniors address and navigate their personal, social and psychological challenges. Common areas of focus include coping with loss of a loved one, loneliness, unemployment, health and family relationships.

Information and Referral
With an understanding of the senior’s needs and concerns, our social service professionals will share with them information about support services and community resources that can benefit them.
We may also refer them to partner agencies to provide relevant support.
Communities of Care (CoC)
We coordinate holistic health and social care services for our seniors by assessing their social-health situation and connecting them to the relevant service providers.
- Active Ageing Programmes: Such as exercise, craftwork, and health talks
- Befriending: Matching stay-alone seniors with other seniors and volunteers who are able to support them emotionally and engage in conversations
- Care & Support Services: Referring frail and vulnerable seniors to home care support such as housekeeping, meal delivery and medical escort services
FAQs
We provide case management services for clients who fulfil these criteria:
- Singaporean or Permanent Resident aged 60 years and above
- Limited or no family support and require continual assistance
- Vulnerable, with self-care issues due to social isolation, depression and/or physical diabilities
- Require some help with Activities of Daily Living**(ADL) and Instrumental Activities of Daily Living (IADL)***
- Resides in Toa Payoh, Woodlands or Marsiling
** The six activities of daily living (ADL) set by the Ministry of Health are mobility, feeding, transferring, dressing, bathing and toileting.
*** IADL includes the six daily tasks: Housekeeping, Meal Preparation, Medication Intake, Shopping, Use of Telephone and Money Management.
Residents below 60 years old would be considered on a case-by-case basis.
