Our Care Corner Active Ageing Centres (AAC) and Active Ageing Centres (Care) offer a range of services that support vulnerable seniors. Located within their neighbourhoods, we provide seniors with timely access to integrated health and social care for them to age healthily and with dignity.

Our centres are also a safe space for seniors to strengthen social connections with their peers, take part in recreational activities and engage in volunteerism.

Ways We Support

Active Ageing Programmes

Our active ageing programmes aims to help seniors lead purpose-driven lives.

  • Exercise programmes: Prevent and reverse physical and mental frailty
  • Social & recreational activities: Engage in activities and excursions (such as karaoke, arts & craft, cooking, and games) to improve overall well-being and stay active 
  • Educational talks and programmes: Equip and upskill the seniors to achieve independence and promote lifelong learning
Befriending and Buddying

Our volunteer befrienders reach out to isolated and homebound seniors through regular home visits and tele-befriending. By engaging with these seniors, we help keep loneliness and social isolation away. 

Being our eyes and ears, our volunteer befrienders help to understand our seniors’ needs so that we can better support them. Some of our seniors are also volunteer befrienders.

Care and Support

Our centres provide seniors with easy access to social support and information about care services. By linking and referring them to the relevant agencies, we help seniors navigate the healthcare and social service systems to receive the care and support they need.

Hot Meal Programmes

For low-income, homebound seniors who are unable to purchase or prepare their own meals, we provide nutritious lunches and dinners during weekdays.

You may also support our work by Donating and indicating your donation intent to Seniors Services. Donations received from 2 June 2023 to 31 March 2024 will be eligible for matching fund by Tote Board, subject to approval.

SeniorsConnect Digital Skills Initiative

To overcome social isolation concerns among seniors especially when they are homebound or not able to visit our centres, we launched the SeniorsConnect Digital Skills Initiative.

Through this programme, seniors are equipped with hardware and digital skills to stay socially connected virtually. Our centres also organise online programmes that allow seniors to remain active and connected to us. 

Community Health Team & Nursing Post

Located within our centres, the community nursing post provides seniors with convenient and regular access to:  

  • Health checks;
  • Health education and health monitoring;
  • Health coaching and activities;
  • Nursing care;
  • Medication advice;
  • Coordination and referrals to other health and social services

We collaborate with healthcare partners like Tan Tock Seng Hospital (TTSH) and Woodlands Health Campus to establish these nursing posts and facilitate the delivery of care services and programmes.

Functional Screening

Functional screening helps to identify age-related decline in vision, hearing and oral health. Early identification and intervention helps seniors to age well in place and enhance their quality of life. 

We collaborate with AIC to provide functional screening across our centres. Please contact our centres to find out about upcoming sessions.

Day Care and Community Rehabilitation

At our Active Ageing Care Hubs, we provide care services, such as day care and community rehabilitation. 

We also provide active ageing programmes such as exercise programmes, recreational activities and educational talks, befriending support and information about care services.
